**Clinical Pharmacist II - Heme/Onc (54137)** + Job Type:Full Time + FTE/Bi-Weekly Hours:1.0/80 + Shift:Rotating + Hours in Shift:8 + Location:Palo Alto, CA + Req:54137 **Job Description** As a member of a multidisciplinary health care team, the clinical pharmacist provides the full range of distributive, clinical, consultative, research, and/or administrative pharmaceutical services. Pharmacist responsibilities include: verifying orders, dispensing medications, overseeing technicians, monitoring patients; serve as members and consultants on treatment or diagnostic teams; disseminate drug information to members of the hospital and clinic staff and interested community groups; may participate in hospital or clinic research projects or perform pharmaceutical research. Pharmacist to provide the above services to oncology/hematology patients in the inpatient setting and ambulatory infusion patients as determined by qualification of candidate. Opportunities available to precept PGY1, PGY2 pharmacy residents and pharmacy students. Additionally, position includes the following responsibilities: working with IT and prescribers for Beacon builds, validation, conversion, and active participation supporting committees as needed to support oncology patient care. Recent (within 5 years) Oncology/Hematology/BMT pharmacy practice experience for at least 2 years and/or completed a PGY2 oncology residency. Ability to work rotating shifts (days/evenings, weekends, holidays) and at various sites within the Palo Alto, San Jose, Redwood City area. Ability to precept pharmacy residents and students.
